Members

— everyone holding a membership in the group. Memberships are assigned per user by an admin on the Users page.

Leads

— the members flagged as leads, so this is always a subset of the member count. A group can have any number of leads, including none, and the flag is per group: someone can lead one group and simply belong to another.

Lead is a label, not a permission. Nothing in core grants leads extra access. An admin sets the flag per member on the Users page, alongside the membership itself.
